Physiocare 2 U is committed to providing quality services to you. This policy outlines our ongoing obligations in respect to how we manage your personal information.

We may also collect health-related information, including medical history, clinical notes, diagnoses, and other information relevant to health services or treatments you have received or may require.

We may disclose your information to other health service providers as necessary. In accordance with privacy legislation, you are entitled to access any information that we hold about you. 

We release some health and billing information to health insurers, third party insurers and authorities to meet their requirements for claim assessment and payment. Your physiotherapist will discuss the information with you and will limit this release if you require. If you ask us to withhold certain information, this may have an impact on your insurance claim. We will only release additional information to other parties, including insurance companies, if you provide permission for the information to be released.

Physiocare 2 U collects your personal information primarily to provide our physiotherapy and related healthcare services to you.

Security of Personal Information

Your personal information is stored in a manner that reasonably protects it from misuse, loss, unauthorised access, modification, or disclosure.
 

When your personal information is no longer required for the purpose for which it was collected, we will take reasonable steps to destroy or permanently de-identify it. However, most personal information will be stored in client files, which will be retained for a minimum of seven (7) years, in accordance with legal and professional obligations.

It is important to us that your personal information is accurate and up to date. We will take reasonable steps to ensure the information we hold is accurate, complete, and current.
